This notice provides you with important information regarding how you may look up complaints, citations, inspections, enforcement actions, and penalties taken against the facility.
There are two ways to access this information:
- Go to the state website maintained by the New York State Department of Health: https://profiles.health.ny.gov/nursing_home/#5.79/42.868/-76.809
Enter the name of the facility in the search box. Once you have located the facility, you may click on the “Inspections” tab to access information regarding complaints, citations and enforcement.
- Go to the Nursing Home Compare Website maintained by the U.S. Department of Health and Human Services: https://www.medicare.gov/care-compare/?providerType=NursingHome&redirect=true
Enter the name of the facility in the search box and click on “search.” Once you have located the facility, you may click on the “Details” tab and scroll down to view inspection results, penalties and other information about the facility.
